ANGELES CITY- Depositors of the padlocked Tarlac-based First Provincial Bank Inc. also called A Rural Bank, have been advised yesterday to claim their deposit insurance in four venues in Benguet, Tarlac, and Pampanga this June.
In a bulletin yesterday, the Philippine Deposit Insurance Corp. (PDIC) said it will start “receiving, processing and paying valid insurance claims” of the depositors starting June 1.
Last April the Bangko Sentral ng Pilipinas (BSP) placed the bank under the receivership of the state-run PDIC.
The First Provincial Bank is a four-unit bank with head office located along McArthur Highway corner Juan Luna St. in Tarlac City.
It has three branches including one in Mabalacat, Pampanga; Paniqui, Tarlac; and La Trinidad, Benguet.
Records showed that the bank had 4,233 accounts with total deposit liabilities of P190.5 million as of June 30 last year.
Securities and Exchange Commission (SEC) records showed the bank is owned by Blesilo Florido Buan, Florido Buan, and Romina Canilao.
The PDIC said all valid deposits shall be paid up to the maximum deposit insurance coverage of P500,000.
It advised depositors with valid accounts with balances of P10,000 and below, who have no outstanding obligations with the bank and who have updated their addresses with the bank in the past year, need not file deposit insurance claims as their payments would be mailed.
The PDIC said it will issue starting today, May 31, “priority numbers” containing the date when the depositor could go to designated venues for their insurance claims. The numbers are to be issued in the same venues.
Depositors who had accounts at the head office of First Provincial Bank in Tarlac will be serviced at the bank premises from June 1 to 13, PDIC said.
Those with deposits at the La Trinidad, Benguet branch will be served during the same period at the Gladiola Convention Center.
The PDIC also said that depositors in the bank’s branch at M.H. Del Pilar St. in Paniqui, Tarlac are to be serviced in the bank premises from June 1 to 5, while depositors of the branch in Balibago, Angeles City also from June 1 to 5 at the Clark Museum at Clark Freeport.
“Claims of depositors will be processed on site and if found valid and supported with complete requirements, will be paid onsite,” PDIC said.
Those who fail to make claims in these dates could still later do so personally at the PDIC home office on the 4th floor of the SSS building at the corner of Ayala Ave. and Rufino St. in Makati City.
The PDIC said depositors have until April 21, 2014 to file their claims. Beyond this, no deposit insurance claims would be entertained.
The PDIC also reminded the depositors to bring the following when making insurance claims: accomplished claim form and claim status sheet, original and photocopy evidence of deposit such as passbook and certificate of time deposit, and original and photocopy of two valid identification card with photo and signature.
Depositors below 18 years old are also advised to submit a photocopy of their birth certificate issued by the National Statistics Office or a duly certified copy from the local civil registrar.
